Community Support/Case Management

Community Support (“Community Psychiatric Rehabilitation”) or case management services provides direct services to adults in their homes, community and each of our office locations. Each person enrolled will be connected with a Community Support Specialist (CSS) who is able to assist individuals with developing the skills needed to live the most productive and satisfying life possible. The community support specialist is there to help you with any goals you may have such as: preventing hospitalization, learning to budget your money, learning about symptoms and medications, ways to manage your mental health symptoms, accessing community resources as well as many others things identified by you and your team.
