Community Support/Case Management
Community support and case management services are part of the Community Psychiatric Rehabilitation Program that provides additional support to persons with more serious mental health conditions. These services are provided by a Community Support Specialist (CSS) who is trained to provide assistance to individuals in developing skills needed to live the most productive and satisfying life possible.
The CSS is there to assist you with goals such as preventing hospitalization, budgeting your money, learning about symptoms and medications, accessing healthcare, improving your health and wellness and accessing needed community resources. The CSS works closely with your other mental health and healthcare professionals to minimize barriers to care.
